    The Diabetes Manual

    Facilitator training for 1:1 patient education behaviour change programme

    Front Cover of Diabetes ManualThe Diabetes Manual is a 1:1 structured education programme for people with Type 2 diabetes. It has been developed in collaboration with the Heart Manual Team and meets the national criteria for diabetes patient education, having:

    • A structured, written curriculum
    • Trained educators
    • Quality assurance processes, and
    • Audit procedures.

    The Diabetes Manual is aimed at people with a new diagnosis of Type 2 diabetes and those whose self-management confidence and diabetes control is weakened and might benefit from a boost. It can be delivered in primary or secondary care by health professionals experienced in providing routine diabetes care. The Diabetes Manual for patients and the facilitator training for health professionals are both based on self-efficacy theory. This social learning theory aims to enhance health professionals’ skills and their patients’ skills, confidently and progressively in developing and valuing new behaviours.

    Course Structure:

    • Two-day Facilitators Course for healthcare professionals (e.g GP or nurse)
    • 260 page workbook to be followed by the person with diabetes
    • CDs x 2 (FAQ and Relaxation)
    • Nurse telephone support
    • Theoretically underpinned using Cognitive Behaviour Therapy and Self-efficacy theory

    Delivery Option

    The course is usually run at Warwick Medical School as a two-day training programme.

    We have also developed the additional option of delivering the course at your PCT for a minimum of 8 people. Please contact the course co-ordinator for costing and additional information.

    The Facilitator Course

    The two-day facilitator training aims to develop the health professionals’ confidence for supporting the patient through the programme. This includes, for example, the professionals’ development of telephone support skills and encouraging patient engagement with the stress management components of the Diabetes Manual. The evidence relating to psychological interventions to promote self-management will be presented and health professionals will learn how to use this combined psychological and educational intervention with their patients.

    Outcomes

    At the end of this two-day Diabetes Manual facilitator course;

    • You will be trained to deliver this structured education and self-management programme with your patients
    • Your facilitation skills will be used when you enable your patients to follow the programme and support them in goal setting and evaluation through your telephone support

    Further copies are available for purchase from Warwick Diabetes Care once the initial programmes have been offered to patients. Only trained facilitators are able to purchase, and use, the Diabetes Manual programme with their patients.

    Prices

    The Diabetes Manual: 2nd Edition is currently being finalised which will affect costings. The final price will include the two-day Facilitators course, hospitality, a number of complete copies of the programme including the Diabetes Manual and CDs. Please contact the Course Co-ordinator for prices.

    Accommodation is not included.

    The Diabetes Manual Workbook

    A randomised control trial of the Diabetes Manual Programme found that it reduced diabetes related anxiety and depression and improved self eficacy (confidence). Participating nurses found delivering the Programme improved their job satisfaction.

    The Diabetes Manual is a 12 week patient-led programme supported by a trained facilitator through a single face to face meeting and 3 telephone support calls. The Diabetes Manual incorporates all aspects of diabetes self-management, including:

    Diabetes Facts Goal setting and evaluation Stress, anxiety and depression
    Smoking cessation Complications Blood glucose monitoring
    Metabolism Medication Weight loss
    Tests Nutrition and glycaemic index
    Cholesterol
    Exercise Patient stories
    Insulin initiation
