APTS week 3 (Durham): requirements form

On the Thursday evening of the APTS week we will be holding an 'Academy Dinner'. Please use the form below to tell us if you plan to attend, and to tell us of any other special requirements you may have: this needs to be done by 17th June please.

During your stay at Durham University, lunches and dinners will be provided. Please complete the following form to specify dietary requirements.

The academy dinner is organised for Thursday night. Please indicate whether or not you will be attending and your menu choice. Please view this decision as a firm commitment. N.B. For the Academy dinner, smart casual dress is recommended.

"Mix and match" of menu components will not be possible. Any choice is always a firm choice for the whole menu.
