Text boxes in forms
Use text boxes in forms to capture routine information such as a respondent's name and contact details, and provide a space for respondents to write free text.
Name | Description |

Address | Capture postal address information in a form: address line 1; address line 2; town or city; county; postcode; country. |
Date | Enable respondents to choose a date from a calendar pop-up window. |
Provide a text box for respondents to enter their email address. | |
File attachments | Enable respondents to include file attachments when they submit a form. |
Instructions | Provide additional comments or instructions to respondents in the body of the form. |
Multi-line text box | Provide a text box for respondents to type an answer. Allow answers over several lines. |
Personal details | Pre-populate a form with a signed-in Warwick member's University ID, usercode, full name and email address. |
Phone number | Provide a text box for respondents to enter their telephone number. |
Single-line text box | Provide a text box for respondents to type a one-line answer. |
